Please complete it and RETURN IT TO THE PRESBYTERY OFFICE ELECTRONICALLY adminassist@pbylakeerie.org OR BY MAIL (2816 Elmwood Avenue, Erie, PA 16508) BY January 23, 2026. This will allow for sufficient time to prepare the report. If we do not hear from you by the 23rd, we will assume that you did not have anything to report.
